Iatrogenic thoracic outlet syndrome caused by revision surgery for multiple subacute fixation failures of a clavicle fracture: a case report.

چکیده
During revision surgical fixation of clavicle fractures and fracture nonunions, compression of the prominent neurovascular structures of the thoracic outlet can be an important potential complication. In this report, we describe the case of a manual laborer who had iatrogenic compression of the thoracic outlet of his dominant arm by fracture callus during revision surgery after multiple subacute fixation failures. The surgeon initially failed to recognize that the anatomic alignment of the clavicle fracture during osteosynthesis had resulted in thoracic outlet syndrome (TOS), as definitively established by venogram 14 days postoperatively. This case illustrates how the constellation of signs and symptoms can be misleading when this problem occurs in a subacute setting. A number case reports have described iatrogenic TOS in the setting of a midshaft clavicle fracture associated with nonunions, malunions, and delayed diagnosis. This report is significant because it addresses a case of iatrogenic TOS accompanied by (1) multiple subacute failures of surgical fixation before final/definitive fixation, all within 60 days of the fracture, (2) description of the patient’s clinical findings and diagnostic studies that ultimately led to a venogram and revision surgery, and (3) quantification of patient function and satisfaction for nearly 5 years to definitively establish whether decompression surgery for TOS would be required.

A Case of Traumatic Thoracic Outlet Syndrome
Thoracic outlet syndrome (TOS) due to thoracic trauma is a rare disorder. Surgical treatment of TOS is especially rare. We report here a case of traumatic TOS caused by right 1st rib and clavicular fracture after a traffic accident. The patient underwent first rib resection and open reduction with fixation of the clavicle through axillary and supraclavicular incisions.
متن کاملLate Thoracic Outlet Syndrome after Clavicle Fractures in Patients with Multiple Trauma: A Pitfall of Conservative Treatment
Late thoracic outlet syndrome is a rare complication of clavicle fractures and usually warrants surgical treatment though its cause of clavicle fractures are a common injury and in most cases are treated conservatively. We report a case of a clavicle fracture accompanying severe multiple trauma, which was necessarily treated conservatively because of the severe associated injuries of the patien...
متن کاملProgressive Brachial Plexus Palsy after Osteosynthesis of an Inveterate Clavicular Fracture
INTRODUCTION The thoracic outlet syndrome (TOS) is a rare complication of clavicular fracture, occurring in 0.5-9% of cases. In the literature from 1965 - 2010, 425 cases of TOS complicating a claviclular fracture were described. However, only 5 were observed after a surgical procedure of reduction and fixation. The causes of this complication were due to the presence of an exuberant callus, to...
متن کاملFemoral Condyle Fracture during Revision of Anterior Cruciate Ligament Reconstruction: Case Report and a Review of Literature
Background: A rare and devastating complication following anterior cruciate ligament (ACL) revision reconstruction is femoral fracture. Case presentation: A 35-year old male soccer player with a history of ACL tear from the previous year ago, who underwent arthroscopic ACL reconstruction and functioned well until another similar injury caused ACL re-rupture. Revision of ACL reconstruction w...
متن کاملTracheoinnominate artery fistula caused by migration of a Kirschner wire.
Iatrogenic injury is an underreported but potentially devastating complication of orthopedic wire migration. We report a 48-year-old man with a tracheoinnominate artery fistula caused by migration of a Kirschner wire that was inserted for fixation of a left clavicle fracture nine years before. Following surgical removal of the wire and repair of both trachea and innominate artery, the patient r...
